SYLVANO TENNIS ACADEMY
NEW STUDENT CHECKLIST
1. Completed and signed registration form.
2. Completed and signed private lesson contract. (Only required if student is taking private lessons.)
3. Two letters of recommendation along with contact information of references. (A current teacher, coach, or former instructor will suffice.)
4. Any medical information or records from allergies to past injuries.
5. A letter signed and dated by parent/guardian if student wishes not to be photographed or have their photograph included in any Sylvano Tennis Academy publication.
6. A list of all past tennis instruction taken within the last two years. Please include academy or training camp names, coaches’ names, estimated dates of instruction, and levels of instruction if any. (i.e. a ladder or training group that the student belonged to.)
7. Bring a great attitude and a willingness to work hard!
