NEW STUDENT CHECKLIST


1.  Completed and signed registration form.

2.  Completed and signed private lesson contract. (Only required if student is taking private lessons.)

3.  Any medical information or records from allergies to past injuries.

4.  A letter signed and dated by parent/guardian if student wishes not to be photographed or have their photograph included in any Sylvano Tennis Academy publication.

5.  Bring a great attitude and a willingness to work hard!

BEGINNER TENNIS

6 - 12 Years Old
Minimum Requirements: PER WEEK*
*2 clinics - 5 clinics (2 hour clinic)
*1 private lesson
